- Referral for the examination is issued by your immediate supervisor according to the instructions available on the Employee Service of the University of Lodz (link: instructions for issuing referrals). The referral should be issued in two copies (one for the Personnel Department and one for the clinic).
- You should come to the examination on an empty stomach between 7:00 - 8:30 a.m. with an ID card and referral for the tests. It is worth taking water and something to eat with you because the whole procedure can last until 12 p.m.
- After entering the building, go to room 205, floor 2. You will be provided with further information there.
- After completing all examinations, you receive a medical fitness certificate for work from an occupational medicine physician.
- Certificate from the occupational medicine physician and the second copy of the referral for medical examinations should be submitted on the same day by 3:00 p.m. to the Personnel Department in the Rector’s Office Building of the University of Lodz, room 10 (Lodz, Narutowicza 68, map: GoogleMap).
- The rest of the documents needed for employment will be issued there.
- It is also possible to register the closest family members (partner, children) for health insurance at the University of Lodz. However, family members must reside in Poland and must have a PESEL number (personal identification number). You should report the desire to register your family for insurance to the Personnel Department, room 10.
